HomeCare 1st Choice in Supporting Independence

Individualised Funding

What is Individualised Funding?
IF is a scheme that allows people with a disability greater flexibility, choice and control over supports that enables them to live everyday lives.

What are the Benefits from choosing IF?
• You can manage your own timeframes
• Have greater input and control into who provides support and care to you
• Enjoy greater choice and flexibility with your support to improve your lifestyle
• Opportunity to connect into new networks

Is Individualised Funding for you?
NASC (Needs Assessment & Service Coordination) will discuss with you your needs and work with you to develop a support plan. Family or Wh?nau can have input into this planning stage.
NASC will inform you of the Host organisations and once you decide they will refer your details on to your chosen Host organisation.
Family and friends can assist you to make choices with IF. Natural supports must be identified by you, IF does not take the place of natural supports.
You can not employ a family member who lives in your home. This is NASC policy and should be discussed with your Needs Assessor.

How does IF work?
Individualised Funding is given to the client in service hours. An identified number of hours will be allocated to you by the Needs Assessor.
These hours will be identified as Core and Flexi hours and the value of the hours will cover your support workers hourly rate, any other expenses that you choose, and the services you purchase from your Host provider VisionWest Home HealthCare.
There are a number of service levels you can choose from, but all clients must have the mandatory service level 1.
